Determined from vaccination chart (recorded in H5D/H5M/H5Y); if nothing is record in the chart, then it is determined from the literal question on Polio. (Since DPT is normally administered at the same time as Polio, the program makes the assumption that if the child received a Polio vaccine, then DPT was administered as well.
